Housing HUB is an assessment process for those experiencing homelessness and a connection to housing programs within the Suburban Metro Area Continuum of Care.

Shelter motel vouchers are available through the State’s Emergency Services Program for eligible individuals experiencing homelessness in Scott or Carver Counties.
Eligible individuals may receive up to 60 days of shelter in local community hotels.
Households receiving this service are provided weekly case management to help them work toward more sufficient housing.
HOPES Portage is a family shelter for families with minor children in Scott County.
The 14-unit building is located in Prior Lake and provides 24/7 staffing, case management, a food pantry, weekly programming, and access to community resources.
Families interested must go through Scott County Housing to be referred.

To access supportive housing programs, a household must go through the Housing HUB and be referred through the Housing Registry.
Eligibility varies depending on the program selected. Programs may serve participants with a documented disability or participants who are age-eligible for programs serving area young adults.
CAP case management services help participants connect to community and mainstream resources, increase income through wages, SSDI, SSI, or other sources, work on budgeting, set personal and professional goals, and stay successful in housing and self-sufficiency.
CAP Property Management
CAP Agency owns 9 buildings with 44 units in Dakota and Scott Counties.
CAP Housing provides light case management and teaches residents how to be a good renter and neighbor.
CAP Agency manages West Creek, an apartment complex in Chaska for 18-29 year olds who have experienced homelessness.
CAP provides 24/7 staffing and case management to the 18 units.
The goal is to increase skills aimed at self-sufficiency and permanent housing.
Prevention Programs
CAP receives funds from the Family Homeless Prevention Assistance Program, FHPAP, to help households struggling with rent in Scott, Carver, and Dakota Counties and who are at risk of becoming homeless. This funding is available to singles and families.

Scott Carver Dakota CAP Agency receives Minnesota Community Action Grant and Community Services Block Grant funds through the Minnesota Department of Children, Youth, and Families – Office of Economic Opportunity.
